Q:

Is it true that bellafill only lasts 5 years? Do you have to have it injected every few years? Or is there a more permanent option for acne scars?

Bellafill is the only FDA approved filler specifically indicated for acne scar treatment. Bellafill injection is performed in combination with subcision and this dual treatment has proven efficacy for acne scars.

The bovine collagen is cleared by the body’s immune response and this process takes about 2-3 months. The PMMA microspheres in Bellafill are not removed, but rather are surrounded by multinucleated giant cells (derived from macrophages) and are locked into place through this process. The PMMA is permanent, but the company can only discuss a 5 year duration because of the 5 year safety study that was performed.

To improve results, microneedle treatments, fractional lasers, non-fractional peels, and PDO threads are additional treatment options.

Acne scars like yours are best handled by a combination approach. A good starting point is subcision (breaking up subcutaneous scar tissue) and injection of a facial filler. An HA filler suck as Restylane or Juvederm would certainly work for acne scars. Bellafill, a hybrid filler consisting of bovine collagen and PMMA is specifically approved for treatment of depressed acne scars.

Another approach that I have found to be very helpful for scarring like yours is the use of PDO threads. This is a form of collagen stimulation therapy and insertion of PDO threads into the dermis smooths in the surface component of acne scars.

Because of your darker skin complexion laser treatments are problematic, but continued series of microneedle treatment would be appropriate.

The best approach would be to find a practitioner who is an expert on handling acne scars.

Q:

I have deep acne scars and i am light skin most are red. I’m wondering what would be the filler that gives most immediate improvement ? Also does any Dr’s in NY do mini facelifts for acne scars? I stretch my cheeks and most flatten so I’m not sure why not many Drs would offer that if that’s what the patient is interested in ? It makes me think that they don’t want us to have good results with mini facelift because then they wouldn’t get as much money for multiple treatments down the road. 🙁

Acne scars are best handled by a combination approach and to a large extent this depends on the type of acne scar. For depressed acne scars, subcision (breaking up subcutaneous scar tissue) and injection of a facial filler is very helpful. Restylane or Juvederm would certainly work for acne scars and Bellafill is specifically approved for treatment of depressed acne scars.

A mini-facelift might be appropriate as well. It really depends on co-existing facial issues. Suffice it to say that a facial surgeon would recommend a facelift if they thought it was indicated.

You mentioned that most of your acne scars are red. This suggests that there may be a role for laser or IPL in management of your acne scarring as well.

The best approach would be to find a practitioner who is an expert on handling acne scars.
